How to Apply for the Canadian Dental Care Plan (CDCP)

Who Can Apply and When?

The Canadian Dental Care Plan (CDCP) is opening in phases. Ensure you meet all eligibility criteria before applying.

Application Schedule

Group When You Can Apply
Aged 65 or older Now
Adults with a valid Disability Tax Credit certificate for 2023 Now
Children under 18 Now
All remaining eligible residents In 2025

How to Apply

Submit a single application for all eligible household members to avoid delays.

Required Documents

  • Social Insurance Number (SIN) (if available for children)
  • Date of birth
  • Full name
  • Home and mailing address
  • List of any government social program dental coverage
  • 2023 tax return and Notice of Assessment for you and your spouse (if applicable)

  • Mozilla Firefox (version 128+)

For Apple users, ensure your system is updated to iOS 13+ or MacOS 10.15+.

If You Can’t Apply Online

Call 1-833-537-4342 (TTY: 1-833-677-6262) to apply by phone or request alternative formats.

Renewing Your CDCP Coverage

CDCP coverage must be renewed annually to confirm continued eligibility.

Renew through your My Service Canada Account.

Applying With Assistance

You can ask a trusted person or a legal delegate to help with your application.

Helping as a Trusted Person

  • Friends
  • Relatives
  • Caregivers
  • Translators or interpreters

You must provide clear verbal consent for them to assist you.

Applying Through a Legal Delegate

A legal delegate can apply on your behalf if they have legal authority documented in:

  • Power of attorney
  • Mandate
  • Trusteeship

Submit original or certified copies of these documents by mail or in person at a Service Canada office.
